Warning over recurrent failings in abuse cases

Too many babies were killed or seriously injured by child abuse because of failings by the authorities that are repeated throughout the country, a report has found.

Overall, the reviewers found that too often agencies vulnerable adolescents treated as adults rather than children, and focused on challenging behaviors rather than their need for support.
